Jason Carriveau, PMHNP-BC, ATC
Provider / Chief Executive Officer
Psychiatric Mental Health Nurse Practitioner, Board Certified, (PMHNP-BC)
Certified Athletic Trainer (ATC)
Jason, a Vermont native, began his career in 2002 as a personal trainer at a local gym, dedicated to helping others achieve their fitness goals. He earned a Bachelor of Science in Sports Medicine from Norwich University and gained extensive experience across Vermont and Massachusetts in areas such as personal training, strength and conditioning, rehabilitation, sports performance, and healthcare management.
In 2016, driven by a desire to deepen his impact, Jason returned to school to pursue a Master of Science in Nursing and became a certified Psychiatric Mental Health Nurse Practitioner. After countless late nights, early mornings, and personal growth—along with an extended internship due to COVID—he graduated summa cum laude, a testament to his hard work and dedication.
After two years at a community mental health center, Jason felt inspired to take the next step in his journey, which led to the creation of “OneBreath Vermont,” a venture aimed at fostering wellness and mental health within the Vermont community.

Chelsea Carriveau
Chief Operating Officer
Chelsea, a proud Vermont native, graduated from Endicott College with a Bachelor of Science in Hospitality and Tourism Management, complemented by a minor in Psychology. While her academic journey may not have been marked by traditional accolades, she excelled as a collegiate field hockey athlete, where she honed invaluable skills in teamwork, camaraderie, dedication, and leadership.
Chelsea later founded her own boutique wedding consulting firm, where she passionately helped couples turn their fairytale visions into reality. Her expertise extended to planning corporate and social events for venues like Bolton Valley and Jay Peak Resort. However, her desire for new challenges led her to a career in healthcare administration at UVM Medical Center.
Despite starting with no healthcare experience, Chelsea's drive for growth propelled her from a practice support specialist to a practice supervisor in just ten weeks. Over the next five years, she managed and supported two Family Medicine practices, overseeing 27 direct reports and fostering a culture of collaboration and excellence.
After starting a family, Chelsea embraced another transformative opportunity as a Healthcare IT Project Manager, where she played a key role in executing large-scale healthcare IT projects for the State of Vermont. For the past seven years, she has dedicated herself to this work, positioning herself for her most significant achievement yet: the launch of OneBreath Vermont, a venture poised to make a meaningful impact within the Vermont community.
